We have had a busy few weeks with several statewide conferences for Case Managers and Adult Family Home Care owners. Additionally, we have scheduled several case manager hospital events for the upcoming months- including Evergreen and Valley Medical. Both the Retirement Connection Guide and CareAvailability.com continue to grow in our reach and web traffic and we are so very grateful.
Don’t forget to join us tomorrow at 11:30 on Zoom for the Transitions of Care bi-monthly Zoom to discuss smoother transitions to/from the hospitals.

The “Transitions of Care” meeting’s goal is to improve communication between case management and community providers, especially related to readmissions and start of care following a hospital visit. Agenda -Case Management Week update, how to drop off and who is participating -Updates on new Case Management Resource Fairs that are scheduled -Hospitals requesting that Referral Agencies be more specific on openings and options available -Communicating better with hospitals when patients are sent there -Quicker turnaround times for assessments at discharge Join Zoom Meeting

In the past month, we have asked for donations for non-profits, asked for volunteers at local events, asked you to “like" our social media pages, and more. Sometimes it feels awkward and uncomfortable to ask others to participate in tasks important to us. If it is important to you, it may also be important to others. When we ask for support, we let others know more about our values. My best friend Krista, who passed away in 2018, taught me that there is no harm in asking others to join in and it greatly increases engagement.
