My kids and I are just returning from a very long vacation. The trip ended on Sunday with us honoring my husband’s final wish by spreading his ashes with friends. He would have been 55 this week. This trip would not have been possible without the love and support of my team. As a small business, you can feel the absence of one
person. They covered my responsibilities to enable this extended family vacation and kept the wheels on our bus last summer as I was saying goodbye. I am always proud of our numerous goals and achievements- but I am honored to do so with such amazing people at my side.

We hope you can join us on *August 13th* for the Transitions of Care Hospital Case Management & Provider Meeting at 11:30 am on ZOOM. The goal of this meeting is to improve communication between hospital case management and community providers. This ZOOM call
provides thirty minutes of engagement with hospital case management staff, followed by 15 minutes of provider discussion if needed.

There are often things that we may be anxious about, but flexing our muscles to try something new is ultimately good for us. Maybe it is public speaking, volunteering for a board position, taking on new responsibilities, or changes to our personal lives and priorities. Feel the fear, but do it anyway. You will learn something new, even if it is not always successful on the first try.
